Please provide information to the receptionist and the provider will return your call as soon as possible. [b]Prescription Refills[/b] Our office needs 24-48 hours to process a refill request. Please call your pharmacy 2-4 days before your prescription is due to run out and they will fax us your request. All prescription refills must be current within the last three months to be considered for a refill. If the refill request is denied you will be asked to make an appointment to see a provider. [b][color=ff0000]In Case of Emergency[/color][/b] If you have an after-hours emergency call 9-1-1 or seek care at the nearest emergency room or urgent care center. [b]Office Policies[/b] Please call in advance to make an appointment, and provide at least 24 hours notice to cancel or reschedule and appointment. We will do our best to arrange a same-day appointment if required. [b]Payment[/b] Payment is due at the time of service unless other arrangements have been made in advance. All insurance co-payments are due at time of visit. We accept many PPO Insurances and Medicare. [b]New Patients[/b] New patients must complete a registration form and health history before or at the time of the first visit. If you have an insurance plan please provide your insurance card to the medical receptionist when you sign in.
